Clinic referrals

How to refer a client to a wheelchair and seating and postural management outreach clinic.

Points to note

  • Your documentation must be complete and with our coordinator at least 2 weeks before the advertised clinic date
  • We do not accept late requests unless there is a genuine and exceptional circumstance for a late submission
  • There is a limit to the number of clients we can see in a clinic: we do not overbook clinics.

Making a referral

Submit your request through ProWorkflow (PWF), along with the relevant documents:

As required:

ENAE205 Manual wheelchair specification form (DOCX, 87 KB)

ENAE206 Power wheelchair specification form (DOCX, 220 KB)
