Documents for service requests

Before making a service request, arrange for the customer and/or vehicle owner to complete our ENAV504 declaration form.

You can find all the forms you need in our 'tools and resources' section.

Find vehicle forms

Have supporting documents saved and ready to upload to your request.  

For example: 

  • quotes for assessment, modification or purchase costs 

  • evidence, e.g. birth certificate, education, employment, NASC form etc.

Step 1. Create an EMS advice request in ProWorkflow and attach the supporting documents. 

How to request EMS advice

Step 2. When you receive the EMS advice outcome summary: Complete the EMS Portal then click on the Enable NZ link to access ProWorkflow.

Step 3. When you receive the EMS assessor section summary PDF: Save the summary to your project in ProWorkflow.
